SPORTS REGISTRATION

The Cle Elum-Roslyn School District Athletic Department has partnered with FinalForms, an online forms and data management service.  FinalForms allows you to complete and sign athletic participation forms for your students. FinalForms saves data from season-to season and year-to-year, meaning that you will never need to enter the same information twice. FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

ALL parents/guardians of student-athletes are required to use FinalForms to register their student(s).

What Info Will I Need for First Time Registration

  • 30 minutes

    • You can start and save if you run out of time

  • Basic Medical History & Health Information

  • Insurance Company & Policy Number

  • Doctor & Dentist Contact Information

  • Hospital Preference

  • Students should use their Cle Elum-Roslyn email address if possible

  • FALL: Football, Cross Country, Girls Soccer, Volleyball, Cheer

  • WINTER: Basketball (Boys & Girls), Wrestling, Cheer

  • SPRING: Golf, Track & Field, Baseball, Softball, Tennis

Walter Strom Middle School

  • FALL (August-October): Football, Cross Country, Volleyball

  • WINTER I (October-December): Boys Basketball, Girls Wrestling

  • WINTER II (January-March): Girls Basketball, Boys Wrestling

  • SPRING (March-May):  Baseball, Softball, Track & Field

REQUIREMENTS

Before a student turns out for any sport, they must:

  • Have a physical on file with the athletic department, this can be done by uploading the document to your FinalForms account. Physicals are valid for two years from the date of examination.

  • Fill out and submit ALL forms on FinalForms.

  • Provide proof of Medical Insurance (on FinalForms "Emergency Medical Authorization" form)

    • Purchase here if your student does not have medical insurance.

  • Purchase ASB Card (MS Fee: $30; HS Fee: $35)

    • Go into Middle School or High School Offices to purchase during operational hours

    • Online Store - ASB Fee

      *If you are a home school student, you can register with FinalForms but will need to contact the Athletic Director in order to fully complete registration.*
